Membership Form
Membership is valid through August 31st of each year and must be renewed each year. A new registration form must be filled out to renew membership each year. Please be sure to completely fill out the form and pay the correct dues; incomplete forms or incorrect payments may lead to problems in accurately processing your registration. Please email a picture to tsasoccer@yahoo.com so we can make your player pass.
Season Registration Forms
You must register for each individual season by submitting a separate registration form during the registration period. Please be sure to complete the form in its entirety (including payment) or you may lose your spot on a team.
